Resumen
Objetivo: Describir un caso clínico de osteonecrosis bilateral del maxilar (Estadio 3) relacionada con el uso de pamidronato disódico (Aredia®) en un paciente oncológico y establecer una breve revisión de la literatura. Detalles del caso: Se trata de un paciente de 78 años en tratamiento paliativo por cáncer de próstata (adenocarcinoma Gleason 7) y diagnosticado con osteonecrosis de mandíbula (ONM) relacionada con fármacos en el tercio medio de la cara. Se realizó tratamiento quirúrgico agresivo combinado con terapia farmacológica adyuvante. El paciente evolucionó favorablemente con resolución completa de los síntomas, ausencia de recurrencia y mejoría en la calidad de vida. Consideraciones finales: La fisiopatología de la ONM no se comprende completamente y contiene muchos factores de riesgo, por lo tanto, aún no se han establecido protocolos de tratamiento definitivos. Es importante señalar que la eliminación del hueso necrótico es importante en el tratamiento de la ONM, siendo las resecciones quirúrgicas procedimientos invasivos y necesarios en pacientes en estadio avanzado, especialmente en pacientes en estadio 3. El protocolo quirúrgico presentado en este estudio muestra resultados prometedores para el manejo quirúrgico de estadios avanzados en pacientes con ONM de alto riesgo, sin embargo, se necesitan más estudios para un tratamiento definitivo y efectivo.
