Abstract
Objective: Report a case of a patient who developed Still’s Disease (SD) secondary to a Chikungunya infection, reviewing differential diagnoses and possible interrelations. Case details: A 21-year-old patient of the female sex, with no comorbidities, presented with acute daily fever, disabling polyarthralgia, evanescent skin rash, and elevated inflammatory markers after a febrile episode consistent with symptomatic arbovirus infection. Initial investigation ruled out community-acquired infections, with negative autoantibodies and unremarkable imaging exams. SD was confirmed by clinical and laboratory criteria, including ferritin > 3000 ng/mL. Positive IgM for Chikungunya indicates a causal relationship. The patient showed a good response to corticosteroid therapy combined with methotrexate, with symptom remission and progressive functional improvement. Final considerations: SD is a rare and challenging inflammatory condition, whose diagnosis relies on careful exclusion of other etiologies. Viral infections may act as immunologic triggers. This case report reinforces the importance of clinical suspicion in prolonged symptoms, the role of multidisciplinary management, and early treatment to achieve a favorable prognosis.
