Abstract
Objective: To report the experience of elaborating an affective medical record aimed at cancer patients treated at the chemotherapy outpatient clinic of an oncology clinic. Experience report: It was carried out by a nursing student who was part of an interdisciplinary theoretical-practical extracurricular internship program for initiation in oncology, in a reference oncological treatment clinic in the North Region of Brazil. In this scenario, it was found that multidisciplinary care for cancer patients is directed towards chemotherapy and symptom relief. Based on a literature search and dialogue with space professionals, these items were selected to constitute the Affective Record: personal data; daily occupancy; interpersonal support network; belief/spirituality; physical lifestyle; subjective aspects of predilection; and drawing space for open chart. Final considerations: The affective medical record has theoretical bases to assist in its elaboration and constitutes a positive tool for humanized health care.
