Resumo
Objetivo: Relatar o caso de paciente com lúpus eritematoso sistêmico (LES) que apresentou hemorragia alveolar difusa de evolução subaguda, evidenciando uma forma incomum de apresentação clínica. Detalhamento do caso: Paciente do sexo feminino, 17 anos, portadora de LES, apresentou quadro respiratório subagudo com evolução de três meses, manifestado por dispneia, febre, dor torácica e alguns episódios de hemoptise. Na admissão, encontrava-se hemodinamicamente estável e eupneica. Exames demonstraram anemia, elevação de provas inflamatórias e consumo de complemento, sem disfunção renal, associado a extensas opacidades em vidro fosco difusas e bilaterais e grave acometimento pulmonar. Após exclusão de diagnósticos diferenciais, estabeleceu-se o diagnóstico de hemorragia alveolar difusa (HAD) associada ao LES. Recebeu pulsoterapia com corticoide e ciclofosfamida, com evolução favorável, sem necessidade de ventilação mecânica e de cuidados críticos. Considerações finais: a HAD subaguda é rara, principalmente quando associada ao LES, reforçando a importância da suspeição diagnóstica em pacientes lúpicos. O manejo oportuno foi determinante para o desfecho favorável.
