Vacters sequence complicated with chylothorax

Abstract

Objective: To report the case of a newborn with a VACTERS sequence, congenital malformation, without prenatal diagnosis and who, due to a surgical approach to the chest, developed chylothorax. Case report: Vacters syndrome or sequence is a set of midline malformations listed in the acromion: vertebral (V), anal atresia (A), tracheoesophageal fistula with or without esophageal atresia (ET), radial dysplasia, cardiac anomalies (C) and limb defects (L). As a consequence, the child may be subjected to urgent postnatal surgical procedures and exposed to complications inherent to the act, such as chylothorax, which consists of the presence of lymph in the thoracic cavity, whose treatment must be early due to the risk of developing respiratory insufficiency. Treatment can be conservative, with a diet rich in medium-chain triglycerides or drug treatment, with the use of Octreotide, a synthetic analogue of somatostin, as a preference to surgical treatment. After recovery from the condition, he was maintained only on a diet rich in medium-chain triglycerides (MCT). Final considerations: It can be concluded about the knowledge of an uncommon pathology that requires early treatment that can be linked to good prenatal care. Furthermore, it is observed that if surgical procedures are necessary, complications can occur and appropriate treatment is implemented.
