Abstract
Objective: To analyze the temporal trend of leprosy in Brazil's most hyperendemic capital, from 2001 to 2020. Methods: Ecological time-series study. Epidemiological and operational indicators of leprosy were calculated from data from the Notifiable Diseases Information System (SINAN) and analyzed using Poisson regression models with inflection points (Joinpoint). Results: Of the 7,165 new cases diagnosed during the period, the overall detection rate showed a heterogeneous trend. There was a significant decrease (APC: -8.1; 95% CI -13.2; -2.7) between 2001 and 2013. From 2013 to 2018, the trend was significantly increasing (APC: 40.3; 95% CI 8.5; 81.5) and in 2019 and 2020 it was stable (APC: -32.1; 95% CI -62.3; 22.3). The detection rate of cases with grade 1 physical disability showed a significant increase (APC: 21.1% 95% CI 2.0; 43.9) between 2001 and 2009 and the annual percentage average remained stable throughout the period. The detection rate with grade 2 remained stable. Conclusion: There was variability in the indicators according to the actions that the municipality adopts as a priority. Operational difficulties in the leprosy control program still persist and contribute to active transmission and hyperendemic status.
