Abstract
Objective: To report the care process, clinical evolution, and the main challenges related to therapeutic adherence. Case details: This is a descriptive study, with data collected through weekly home visits over a period of 5 months and 22 days, complemented by clinical evaluations at a university health unit. The patient, a 70-year-old male, presented with frequent hyperglycemia, irregular use of medication, and a history of previous amputation. The therapeutic approach involved the use of calcium alginate with silver, collagenase, and later silver sulfadiazine, applied according to the wound’s progression. A significant improvement of the lesion was observed with progressive adherence to the guidelines, especially to glycemic control, although metabolic instability remained a constant challenge. Final considerations: The case highlighted the importance of strict metabolic control, systematic foot care, and the active involvement of the patient and his family. It also emphasized the role of nursing and extension activities in health education and the prevention of complications in people with type 2 diabetes mellitus (T2DM).
