Abstract
Objective: To evaluate the demand, actual utilization, and stock retention of blood components reserved for elective surgeries at the transfusion service of a maternity hospital in northern Brazil, describing indicators and assessing adherence to the institutional transfusion protocol as a model of good practices within the Brazilian Unified Health System (SUS). Methods: A retrospective study analyzing monthly records of blood component reservation, utilization, and stock retention from January to December 2024, including laboratory parameters of hemoglobin and hematocrit in transfused patients. Results: Of the 2,714 blood units requested, 547 (20.2%) were reserved for elective surgeries, with effective utilization of 31 units (5.7%) and 94.3% remaining in stock. Proportion testing demonstrated that the effective utilization rate was statistically lower than 20% (p < 0.0001). The mean hemoglobin level in transfused patients was 9.12 g/dL, with no statistically significant difference compared with the clinical cutoff of 9 g/dL (p = 0.764). Conclusion: High adherence to the institutional transfusion protocol was observed, with rationalization of surgical blood reservations while maintaining patient safety. These findings indicate a positive impact on blood component management and reinforce the potential applicability of this model in other public healthcare services within the SUS.
