Abstract
Objective: Describe profound hypothermic circulatory arrest (Deep Hypothermic Circulatory Arrest - DHCA) in aortic injuries, discussing resulting pathophysiological changes and strategies for reducing injuries to target organs. Bibliographic review: Anesthetic management of patients with complex aortic injuries represents one of the greatest challenges in cardiovascular surgery. In these situations, the need for strict control of the operative field and protection of vital organs, such as the brain, necessitates advanced perfusion and organ protection techniques. One such technique is DHCA, used to temporarily halt circulation without causing irreversible damage to the central nervous system. This technique requires rigorous anesthetic planning and intensive monitoring to mitigate risks and ensure the patient’s hemodynamic and metabolic stability. Final considerations: DHCA is a crucial technique in repairing aortic injuries, providing neuroprotection to the central nervous system during the procedure. However, despite these advancements, DHCA still involves considerable challenges, such as hypothermia-induced coagulopathy and the risks inherent in prolonged use of extracorporeal circulation, which require special attention and appropriate management strategies.
