Abstract
Objective: To report two cases of mitral valve insufficiency resulting from chordae tendineae rupture. Case details: Observational and descriptive study of two clinical cases of male patients, aged 44 and 57 years, who sought specialized medical attention due to symptoms consistent with mitral insufficiency. The patients were diagnosed through transesophageal echocardiography performed during hospitalization, which confirmed the presence of severe mitral insufficiency associated with chordal rupture. Subsequently, they underwent distinct surgical procedures: the first patient, aged 44, underwent mitral valvuloplasty, while the second, aged 57, required mitral valve replacement. The clinical outcomes were notably different. The 57-year-old patient developed cardiogenic shock in the postoperative period; in contrast, the second patient had a satisfactory recovery, with no significant complications in the postoperative period, and was discharged with recommendations for rheumatic fever prophylaxis. Final considerations: It can be considered that semiology is a fundamental tool in identifying patients with symptomatic or oligosymptomatic mitral valve disease and that their treatment should be individualized according to the degree of valvular alteration and associated conditions.
