Immediate implant with customized healing cap to maintain peri-implant esthetics

Abstract

Objective: To describe the installation of an immediate implant, with a personalized healing cap, for the maintenance of peri-implant esthetics, when immediate loading is contraindicated. Case details: The patient presented at the clinic with a fracture of element 21, being subjected to minimally traumatic extraction, installation of an immediate implant, with a torque of 25Ncm, and adaptation of a temporary abutment to promote healing of the gingival tissue during the period of osseointegration. A provisional removable partial denture was adapted so as not to overload the implant. After the period of osseointegration, the gingival contour was stable, with no change in the levels of the papillae or retraction of the peri-implant mucosa, thus allowing the definitive molding and cementation of the single porcelain crown. Final considerations: It  was considered that in cases of contraindication to the application of immediate load, the use of a customized healing cap in the esthetic region, immediately after implant installation, has the main advantages of tissue sealing, maintaining the profile of gingival emergence and favoring definitive rehabilitation, without application of early loading that could influence implant loss in the short term.
