Cuidado pré-natal de paciente com angioedema hereditário

Resumo
Objetivo: Discutir a profilaxia com inibidor de C1 (C1-INH) por via endovenosa, antes da indução anestésica para o parto em gestantes, para evitar crises do angioedema hereditário (AEH) tipo III. Detalhamento de caso: Primigesta, 33 anos, relata história de edema facial desfigurante, recorrente com surgimento após a adolescência. No primeiro trimestre evoluiu sem crises e as ultrassonografias apresentaram resultados morfologicamente normais até o segundo trimestre. No terceiro trimestre apresentou crescimento intrauterino restrito tardio (CIUR tardio) identificando no feto o peso percentil 3 e com o doppler da cerebral média no limite inferior da normalidade. Realizou-se cesariana, com prescrição do C1-INH antes da indução anestésica. O parto transcorreu bem, a paciente e o bebê receberam alta após 48 horas de internação. Nos exames identificaram C4 no limite inferior da normalidade, com C3 normal e C1 normal. A classificação da paciente foi AEH tipo III e ela segue em controle e mantém-se assintomática. Considerações finais: A grande preocupação do angioedema hereditário tipo III é que em geral, as dosagens de complemento podem estar normais em pacientes assintomáticos e pode apresentar como gatilho o estresse e trauma. Portanto, a profilaxia com C1-INH por via endovenosa, antes da indução anestésica, pode mostrar-se importante estratégia para o controle das crises.
