Profilaxia de úlceras de estresse em pacientes internados e complicações associadas ao uso de Inibidores da Bomba de Prótons

Resumo

Objetivo: Avaliar a eficácia da profilaxia de úlceras de estresse com inibidores da bomba de prótons (PPIs) em pacientes internados em unidades de terapia intensiva (UTIs) e investigar as complicações associadas ao seu uso. Métodos: Foi realizada uma revisão integrativa seguindo a estratégia PVO, analisando artigos publicados entre 2014 e 2024 que abordaram a eficácia dos PPIs em pacientes críticos. A pesquisa foi realizada na base de dados PubMed, resultando na seleção de 19 artigos relevantes. Resultados: Os achados indicam que, embora os PPIs reduzam a ocorrência de hemorragias gastrointestinais em pacientes de alto risco, não demonstram impacto significativo na mortalidade. Além disso, o uso prolongado de PPIs está associado a complicações, incluindo infecções hospitalares e aumento do risco de mortalidade. A literatura revela controvérsias sobre a necessidade de profilaxia universal com PPIs e sugere a importância de estratégias de tratamento individualizadas baseadas em risco. Considerações finais: O uso indiscriminado de PPIs para profilaxia em UTIs deve ser reconsiderado, levando em conta os potenciais riscos e complicações envolvidas. Estudos futuros são necessários para estabelecer diretrizes mais direcionadas para a utilização desses medicamentos em populações críticas.
