Seminoma primário de mediastino

Resumo

Objetivo: Descrever o diagnóstico e o manejo terapêutico de um paciente com seminoma extragonadal de origem mediastinal, destacando as particularidades da condução deste tipo de tumor. Detalhamento de caso: O caso relatado descreve um homem com seminoma mediastinal, apresentando dispneia, dor torácica e disfagia. A tomografia revelou massa mediastinal extensa, enquanto a imunohistoquímica confirmou seminoma metastático. O tratamento oncológico foi o esquema BEP (Bleomicina, Etoposide e Cisplatina) por quatro ciclos. O paciente completou os quatro ciclos de quimioterapia, com boa tolerância, houve redução significativa da massa tumoral e melhora dos sintomas, apesar de persistirem lesões ósseas e pleurais. Um ano após o diagnóstico, o paciente está assintomático, aguardando decisão terapêutica. Considerações finais: O paciente recebeu a terapia padrão de abordagem a seminomas, de acordo com a Sociedade Europeia de Oncologia Médica, que consiste em poliquimioterapia com bleomicina, etoposide e cisplastina, sendo recomendado 3 ciclos para pacientes categorizados como bom prognóstico  e 4 ciclos para aqueles que apresentarem prognóstico intermediário. Por fim, a resposta favorável à quimioterapia à base de cisplatina observada está em conformidade com o tratamento de primeira linha recomendado para seminomas.
